Abstract: 

Background: The progression of atherosclerosis, which is the main cause of many heart surgeries, find it far more quickly with diabetes because diabetes is the production of c-peptide that causes increased permeability is increased vascular Intimal layer regresses. Conclusion: Considering the high prevalence of Helicobacter pylori in people working with clinical signs of enteritis, gastric infection, it can be stated that Helicobacter pylori has an important role in causing enteritis and as a carcinogen in the studied samples. Due to the presence of bacteria that can be transmitted to others, it is necessary to identify and treat human carriers.

Abstract: 

Background: The prevalence of obesity is increasing in many parts of the world. Impaired glucose metabolism is strongly associated with body weight. The aim of this study was to compare effect of three methods of bariatric surgery (gastric bypass, adjustable band and sleeve gastrectomy) on insulin resistance in patients with morbid obesity.Methods: This intervention study was performed on patients with morbid obesity referred to obesity surgery clinic of Razi Hospital in Ahvaz 2011-12. Patients according to type of surgery (gastric bypass or other procedures) enrolled in two groups. insulin levels, fasting glucose, glycosylated hemoglobin, and insulin resistance were compared between groups before, 3 weeks and 2 months after surgery.Data were analyzed by independent t tests and repeated measures test of SPSS #18 software.Results: in total, 15 patients (2 male, 13 female) were enrolled. patients in the gastric bypass group showed a greater reduction in insulin resistance compared to the other groups (p>0.001), While the comparison of weight loss between the two groups was not statistically significant (p>0.05).Conclusion: Mechanisms of short-term reduction in insulin resistance in patients undergoing gastric bypass surgery are not only related to weight loss and other factors are may be involved.

Abstract: 

Background: The risk of kidney injuries following liver transplantation surgery is high. Aminophylline, a non-specific adenosine receptor antagonist, has been suggested as a preventive measure against post-operative kidney injury. Methods: This double-blind, randomized clinical trial involved 100 patients divided into two groups: Group A (intervention group, 50 patients) received aminophylline, while Group B (control group, 50 patients) received standard care. Variables were measured pre-and post-operatively for up to 5 days, and the results were analyzed. RESULT: The use of aminophylline was associated with a 2% reduction in the incidence of kidney failure, a 12% reduction in severe kidney function loss, and a 12% reduction in moderate kidney function loss compared to the control group. However, no statistically significant difference between the intervention and control groups regarding KDGIO (Kidney Disease: Improving Global Outcomes) levels was observed. The mean serum AST levels on the fourth and fifth postoperative days, hospital stay duration, and IVC clamp time were lower in the intervention group (p < 0. 05). Discussion and Conclusion: The results of this study suggest that, despite the positive effects of aminophylline on GFR (glomerular filtration rate), it does not significantly reduce kidney failure based on the updated KDGIO criteria.

Abstract: 

Background: The idea of preoperative overnight fasting was challenged and verified to have no benefits compared with drinking clear liquids up until 2 hours before surgery. In this study, we explored the possible effects of preoperative oral carbohydrate loading (OCL) on insulin resistance and the clinical outcomes of patients undergoing cardiac surgery. Methods: Totally, 260 patients were recruited and allocated randomly to intervention (n=130) and control (n=130) groups. The intervention group received 250 mL of an oral carbohydrate fluid containing 25 g of glucose, and the control group received standard care. insulin resistance, glycemic indices, and clinical outcomes were assessed before and after surgery. Results: Totally, 107 patients in the intervention group and 103 in the control group completed the study and were, thus, included in the final analysis. Preoperative OCL 2 hours before surgery improved postoperative fasting blood sugar and clinical outcomes after cardiac surgery including thirst, hunger, anxiety, pain, the length of stay in the hospital, and the length of stay in the ICU (all Ps<0. 05). Conclusions: OCL administered preoperatively seems to be beneficial in improving the biochemical and clinical outcomes of patients undergoing cardiac surgery. Notably, preoperative OCL as a safe, simple, and cost-benefit approach is associated with no or negligible harm and, therefore, could be recommended in the setting of cardiac surgery, with careful attention to contraindications.

Abstract: 

Introduction: Renal transplantation is the best method of treatment for renal failure and anesthetic management during this procedure is of great importance. Proper dosage of the anesthetic drug for maintaining the respective depth is crucial as well. Arterial unclamping following anastomosis between the donor and recipient’s kidney vessels, during which the functional kidney is inserted in the recipient’s circulation, is a critical step in transplantation.Objective: To investigate the changes in Bispectral number and pupil dilatation as confirmed and probable markers of anesthesia depth have been investigated.Materials and Methods: This descriptive-analytic study was conducted on 25 patients undergoing renal transplantation in Razi hospital. Alteration in the depth of anesthesia was analyzed using BIS (Bispectral) index (at unclamping moment and its highest value within five minutes following unclamping) as well as pupil dilatation (5 minutes after unclamping). All the patients underwent identical method of general anesthesia. Data were analyzed using SPSS version16, paired t-test and Mann-Whitney U test.Results: Mean and SD value of the maximum BIS number within 5 minutes after arterial unclamping (68/32±9/13) and its value at unclamping moment (59/48±10/65) were significantly different (P<0.0001).The pupil became wider in 84% of the patients immediately after unclamping. No significant relationship was found between the pupil size and BIS change.Conclusion: Considering the present findings, it seems that the depth of anesthesia declines after arterial unclamping following vascular anastomosis of the transplanted kidney. We also found a new sign in kidney transplantation anesthesia, introduced as “PUPIL SIGN”, that happens prior to urination from the transplanted kidney, which could have clinical implications for early prognosis of transplantation success.

Abstract: 

Morbidity and mortality rates are increased due to obesity after organ transplantation; in this regards, bariatric surgery (BS) is believed to be an effective treatment for posttransplant obese patients. Nevertheless, some studies are doubtful in terms of the effectiveness of BS, the most suitable bariatric procedure, and management of immunosuppressant drugs in some kinds of organ transplants. We evaluated nonsurgical therapies, weight reduction, adjustment of immunosuppressants, comorbidities, and the recommended surgical procedures for posttransplant BS for different types of organ transplantations.

Abstract: 

Background: : Nosocomial infections are the main health problems in the world. Prevention of Surgical Site Infection in surgery is the first concern. Wound infection causes increased mortality and length of hospital stay and increase costs in patients treated. Antibiotic prophylaxis in c surgery may be recommended to prevent postoperative complications. Several therapeutic protocols based on international recipes used by cardiac surgeons due to antimicrobial prophylaxis. The aim of this study was to evaluate the clinical outcomes of prescribing antibiotic prophylaxis in patients undergoing surgery in Intensive Care Center is the shariyar of Tehran Research Method: And future prospective cross-sectional study of 200 patients undergoing since entering the ICU until discharge and up to a month after discharge were evaluated in terms of clinical outcomes. Wound infection, renal, pulmonary complications, the incidence of mortality, length of stay in ICU and hospital compared and evaluated based on the type antibiotics. Information recorded and collected self-made Check list. And using descriptive and analytical statistics and were analyzed using SPSS 18. Results: The average age of the subjects Bvd. az ±, 61/32 years patients (139 male ). Received significant relationship between different patterns of antibiotic prophylaxis administered and renal complications, pulmonary complications, wound infection, mortality rate, average length of stay in hospital and there was no special section 61 patients (30/1. were female. lly significant. Average length of stay in intensive care unit patients who received antibiotics were ceftriaxon significantly lower than patients who had not received antibiotics. (pv=0/021) Conclusion: Use different patterns of use and administration of antibiotic prophylaxis in patients undergoing t surgery is associated with different clinical outcomes of use of ceftriaxon in a special section was associated with reduced hospital stay.

Abstract: 

Despite advances in medical treatments, obesity continues to rise at an alarming rate. Bariatric surgery has emerged as the most effective intervention for managing morbid obesity and its associated co-morbidities. Among these surgical methods, laparoscopic sleeve gastrectomy has proven particularly successful in addressing significant weight loss and improving conditions such as hyperlipidemia, diabetes, hypertension, and sleep apnea. This article highlights the importance and effectiveness of sleeve gastrectomy in a unique case involving a patient with a history of kidney transplantation, demonstrating its potential benefits in patients with complex medical backgrounds. The findings underscore the vital role of bariatric surgery in the comprehensive treatment of obesity and related conditions, especially in high-risk individuals.
